​About Take Control Take Charge

Take Control Take Charge is a unique, self-management program funded by the Ministry of Health and Long Term Care, through Hamilton Health Sciences. We offer FREE Workshops to people experiencing and living with Chronic Pain and Chronic Disease. Our programs have been helping thousands of people for over 15 years, throughout the Hamilton, Niagara, Haldimand, Brant and Norfolk Counties.

These workshops will provide you with the tools that can help you to manage your condition and to live a better quality of life.. You will be in a classroom with people just like you! It’s a friendly, non-judgmental atmosphere and a great place to learn and share with other people. You are not alone.

Each workshop runs 6 weeks for 2 1/2 hours per week and will educate you on treatments that may be available to you and what to do when symptoms like difficulty sleeping, seem to take over your life. With the tools and new knowledge you receive from these programs, you can Take Control Take Charge!

Our workshops normally take place at a location near you, but due to COVID-19 Protocols, we are hosting Virtual Online Zoom Self-Management Workshops. Simply go to the ZOOM WORKSHOP tab on this website and follow the prompts to register. 

Workbooks will be mailed to your home 1 week prior to the Workshop, followed by an invitation to the Workshop link sent to your email. All you will need to participate in a Workshop is a device (cell phone, tablet, laptop, or desktop computer) and an internet connection, so you can regain control of you!

We encourage family members and caregivers to join with you. This is a great way for people to understand and support you along the way.

Chronic Pain or Chronic Disease can be diagnosed and experienced at any stage of life. The sooner you start to understand ways to effectively manage your symptoms, the sooner you can “take charge” of your life.  Be proactive, and get the tools to be your own advocate.

Online Chronic Self-Management Program

FREE to individuals across Ontario who are living with one or more chronic conditions such as diabetes, chronic pain, high blood pressure, stroke, cancer, heart or kidney conditions. Many of the workshops are also beneficial for caregivers and anyone looking to improve their health literacy. The online programs can be accessed from any device with an internet connection and have been designed to meet different needs; from one-on-one health coaching and group-based workshop series, to one-time health literacy webinars.
